A Community Care Hub Operational Platform for Counties and Coalitions

A community care hub is a centralized entity that supports a network of community based organizations, handling the shared functions that are wasteful to duplicate: common intake, referral routing, contracting support, and outcome reporting. Wellbeing Care Community (WCC) by eTransX is the technology platform underneath that model. It brings five functions into one system: care coordination, referral management with closed loop tracking, community information exchange, client engagement, and social capital networking. WCC supports counties, health and human services agencies, regional coalitions, and CBO networks.

Wellbeing Care Community

What is a community care hub?

Your community already has the partners. Housing, behavioral health, food access, employment services, peer support, faith communities, schools, clinics.

What it may not have is a way for those partners to see the same person at the same time.

A community care hub solves that at the organizational level. One entity acts as the coordination point for a network of community based organizations, managing a shared directory, a common intake, referral routing, contracting and invoicing support, and reporting back to funders and payers.

Member organizations stay focused on delivering services instead of each building their own administrative and data infrastructure.

The model holds together when the technology underneath it is genuinely shared. Without a common record, a hub becomes another coordination layer that still runs on phone calls, spreadsheets, and follow up emails nobody has time to send.

How It Works

The Community Care Hub Model Works on Three Levels

One record for the person, room for every partner to work their own way, and a live view of the whole network for the people accountable for outcomes.

The Person Level

Each individual or household has one record, one set of goals, and one support team. Progress is tracked against life outcomes, not only against services ordered.

The Partner Level

Each participating organization keeps its own intake questions, workflows, assessments, and interventions. Nobody is forced onto a single agency's process as the price of joining the network.

The Community Level

Hub leadership sees the whole network: where referrals are flowing, where they are stalling, which services are oversubscribed, and which outcomes are improving. That view is what funders and payers ask for, and it is the hardest thing to assemble after the fact.

Built for Coordination

Five Core Functions in One Care Coordination Platform

1

Care Coordination

WCC supports building and managing goal driven personalized success pathways for individuals and families. A pathway includes specific goals, the steps to reach them, metrics to measure progress, and task assignments given to authorized support team members. Coordinators can assign, delegate, or transfer tasks across the team as caseloads and staffing change.

2

Referral Management with Closed Loop Tracking

A closed loop referral is one where the sending organization learns what happened after the handoff: whether the person was reached, whether the service was delivered, and what the result was. WCC generates referrals to organizations in the network, lets those organizations respond inside the system, and sends automatic alerts when an intervention is not completed within the expected window.

3

Community Information Exchange

WCC includes a built-in Enterprise Master Person Index (EMPI), so a record in one partner system can be matched to the same person in another. Administrators define and tune the matching rules for their own network, tightening or broadening the criteria as needed. Community information exchange is what turns a list of partners into an actual shared record.

4

Client Engagement

People reach the hub on their own terms through a client web portal and mobile app, integrated SMS text messaging, and live video. The mobile app includes an optional SOS function that triggers immediate outreach from care team members or a local crisis center.

5

Social Capital Networking

WCC supports assessing and strengthening a person's relationships and sense of belonging by connecting them with family members, mentors, congregations, civic groups, and other authorized support team members. Wellbeing is not only a services problem, and the platform is built to reflect that.

Definition

What Is a Community Care Hub?

A community care hub (CCH) is a community-focused organization that acts as the administrative and technical backbone for a network of community-based organizations. Rather than each nonprofit negotiating separately with health plans and health systems, the hub holds the contracts, manages centralized intake and referral, handles billing and reporting, supports quality and compliance, and maintains the shared technology all network partners use. It is the entity that makes a loose collection of community organizations contractible and accountable as one network.

That backbone role only works if the technology underneath it can carry referrals, consent, eligibility, service records, and outcome data across organizations that use entirely different systems. The Wellbeing Care Community platform provides that layer, supporting network governance, closed-loop referrals, shared case management, and reporting back to payers and funders. The operational realities of the model are covered in maximizing community care hub impact, and the collaboration model behind it in building a hub for collaborative care.
