Going the Extra Mile To Maximize Community Care Hub Impact

A Breakthrough Platform for Maximizing Health and Wellbeing Outcomes  

The momentum around Community Care Hubs (CCHs) is growing, and for good reason. As highlighted in the latest insights from USAging’s Community Care Hub National Learning Community (NLC), community-based networks are uniquely positioned to connect health care with essential social services like housing, food, and workforce development. But until now, the selection of information systems platforms to support successful community care hubs has been limited – focusing on basic closed loop referral systems, case management systems, and activity checklist systems.  

While all of these existing systems support critical functions for community care hubs – they are missing critical components that limit their effectiveness to truly move the needle on improving health and well-being outcomes for the individuals being served.

This focus is on improving personal outcomes and reaching personal goals – not managing cases, not managing electronic medical records, managing referrals, or managing pathway checklists.  

All essential tools in one fully integrated toolbox  

The WCC system includes the ability to support the traditional community care hub functions of managing cases, making closed loop referrals, managing appointments, and providing pathway checklists – but goes the extra mile in providing three additional critical components that community care hubs must purchase separately. These additional included components are: 

  • A robust Community Information Exchange (CIE) engine with a proven track record of 20 years 
  • A robust Client Engagement engine that supports secure two-way texting, emails, video visits, remote assessments, and online educational materials
  • A robust social capital engagement engine that supports secure real time engagement with community health workers, social workers, navigators, friends, and family members beyond traditional clinical professionals.

All of these functions are integrated into one complete system making it much easier to deploy, learn, and use rather than trying to buy and integrate additional separate systems to perform these functions. 

At the same time, the built-in CIE engine includes the new breakthrough XRunner data exchange capability that allows the WCC system to easily and securely share data between existing community-based systems – including any EHR system, HMIS systems, case management systems, and HIE systems in the community.

Cross-Sector Community Level Integration  

The WCC platform enables robust cross-sector community-level integration across health, social services, workforce programs, housing services, transportation services, substance use disorder services, employment services, and more. By providing community social service providers, non-profits, clinical case managers, and government agency workers with a shared community-based platform helps the “right hand know what the left hand is doing”, and reduces the need for the clients being served from having to tell their same story multiple times. More importantly, connecting all team members with one system helps to minimize individuals or families falling through the cracks.
