Making Community Resources Easy to Find and Connect

myWellbeingDirectory is a public-facing community resource portal that makes it easy for anyone to search for local services by ZIP code, connect with agencies in their area, and submit help requests online.

WHAT IS MYWELLBEINGDIRECTORY?

A Community's Front Door to Finding Help

myWellbeingDirectory is a companion product to the eTransX Wellbeing Care Community (WCC) platform. It serves as a branded, publicly accessible web portal where anyone in a community can search for social services without needing to log in, call an office, or navigate a complicated system.

Housing assistance, employment support, food programs, transportation, behavioral health, substance use recovery services and more are all searchable in one place.

Unlike a static resource list or a one-way referral system, myWellbeingDirectory is action-oriented. When a person submits a request, a new client record is automatically created in WCC and routed to the appropriate care manager.

A directory search becomes the start of a care coordination workflow, not a dead end.

BENEFITS

What myWellbeingDirectory Does for Your Community

myWellbeingDirectory is designed to benefit three groups simultaneously: the individuals seeking help, the agencies providing services, and the community systems trying to coordinate it all.

Removes Barriers to Accessing Help

No phone calls, no office visits, and no login required. Individuals can find and request services on their own terms, at any hour, from any device.

Simplifies Client Intake for Agencies

Intake and initial screening happen through the portal, reducing staff time on intake calls and eliminating duplicate data entry between directory tools and case management systems.

Closes the Referral Loop

Because myWellbeingDirectory feeds directly into WCC, referrals do not get lost. Care managers see new requests immediately and can track progress through the full care coordination lifecycle.

Accelerates Time to Care

The automated handoff from portal submission to WCC intake eliminates the lag that typically occurs between a person requesting help and an agency opening a case.

Customizable for Your Community

Communities can brand the portal with their own logo and colors, post scrolling announcements, manage CBO listings, and define which service categories appear.

Supports Reporting and Accountability

Every request submitted through myWellbeingDirectory becomes trackable data inside WCC, supporting outcome reporting, grant compliance, and community-level analytics.

About myWellbeingDirectory

What is myWellbeingDirectory?
myWellbeingDirectory is a public-facing, ZIP code searchable community resource portal built as a companion product to the eTransX Wellbeing Care Community (WCC) platform. It allows anyone in a community to search for local services, view agency listings, and submit help requests online without needing to call, create an account, or visit an office.
How does myWellbeingDirectory connect to the WCC platform?
When an individual submits a service request through myWellbeingDirectory, a new client record is automatically created inside WCC and the referral is routed to the appropriate agency's care manager for review. This eliminates manual data re-entry and ensures no referral is lost between the public portal and the back-end care coordination system.
Does a person need to create an account to use myWellbeingDirectory?
No. myWellbeingDirectory is publicly accessible without login. Individuals can search for services and submit help requests with only their basic contact information, reducing the friction that often prevents people from seeking help in the first place.
Can myWellbeingDirectory be customized for our community?
Yes. Communities can brand the portal with their own logo and colors, configure scrolling announcements, manage their agency listings, and define which service categories appear. Each deployment reflects the specific community it serves.
Can myWellbeingDirectory work alongside an existing 211 directory?
Yes. myWellbeingDirectory is designed to complement existing community resource systems, including 211. The WCC platform can also interface with 211 community directories, allowing communities to build on infrastructure already in place rather than replace it.
Is myWellbeingDirectory HIPAA-compliant?
Yes. myWellbeingDirectory operates within the eTransX Wellbeing Care Community ecosystem, which is built for HIPAA compliance and 42 CFR Part 2 regulatory requirements. All client consent management and data handling follow these standards.
What types of services can be listed in myWellbeingDirectory?
Any community-based service can be listed, including housing and rental assistance, food programs, employment and workforce development, transportation, behavioral health, substance use disorder recovery support, educational services, consumer services, and more.
