The community-based system can be used to collect information on a person's/family's unique circumstances and strengths using assessments. That information is the used to help build and manage a personalized success pathway (action plan) that would guide and support team members making referrals and service orders through coordinated care coordination software. Automated alerts, follow-ups, and client engagement tools like our mobile apps and SMS texting built-in to WCC help connect and guide clients along their own personal growth journey.
